^*I/we agree/understand that despite all due care and efforts by Restaurant Sat Bains hazards such as slips, trip, hot surfaces and liquids will be present in the kitchen as they cannot be eliminated.

I/we will follow instructions given in the interests of safety and will not interfere with any equipment present in the kitchen.

Accordingly I/we agree to release Restaurant Sat Bains and its employees from any criminal proceedings and all civil claims for damage, losses or personal injury resulting from my presence in the kitchen.
